註釋On August 21, 1995, Flight ASA 529 took off from Atlanta, Georgia, headed for Gulfport, Mississippi. Shortly after take-off there was a loud bang. Out of the left side windows, some of the 26 passengers on board could see the propeller blades broken and lodged into the wing and the engine stripped bare of its metal skin. The plane was now effectively flying on one wing! The flight crew desperately tried to keep the plane airborne, but was forced to make a crash landing. Miraculously, all the passengers and crew survived the crash. But within seconds, a raging fire broke out, forcing the passengers to run through searing flames. Many died from their burns. The National Transportation Safety Board (NTSB) investigating the crash determined the propeller failure was caused by corrosion, the third such incident involving the same manufacturer. Their inquiries led them to conclude that the crash could have been avoided had a fatal flaw not been inadvertently covered up.
