The purpose of the Inquiry was to examine and make recommendations for accident prevention and the improvement of systems and procedures related to compliance with, and enforcement of, existing health and safety legislation at the mine. Specifically, it examined training and experience of staff at all levels; the Company's health and safety policy, including the role and functioning of the Joint Health and Safety Committee; the Company's Internal Responsibility System, including internal policies and procedures; internal communications among managers, and between managers and employees; accident and incident investigation, analysis and follow-up; and compliance with, and enforcement of, the Occupational Health and Safety Act, regulations and orders. This document contains the report of the Inquiry.
