註釋This report presents the results from an exhaustive investigation into the crash of Air Ontario flight 1363 at Dryden, Ontario on March 10, 1989 and into the aviation system that allowed it to occur. Evidence was taken under oath in a public forum, subject to cross-examination, from 166 witnesses for a total of 168 hearing days. This report is a synthesis of the testimony of the witnesses and of the contents of the 1,343 documentary exhibits. The report covers the facts surrounding the Air Ontario flight, the Dryden Municipal Airport, and Air Ontario facilities; weather information in Dryden, Winnipeg and points between; events surrounding the takeoff, the flight, and the crash and response; the fire-fighting and rescue services; the investigation process and analysis of the aircraft crash; details on the type of aircraft, its performance and flight dynamics; a history of the Air Ontario management and operation; the role of Transport Canada and its staff and the effects of deregulation; human factors; and legal and other issues.
