It is characterized by noncaseating epithelioid cell granulomas in the absence of other granulomatous diseases such as tuberculosis, fungal infections, autoimmune processes, or delayed-type hypersensitivity reaction to foreign antigensThe complexity of sarcoidosis can be explained by its multiple clinical manifestations that not only involve organs such as lungs but also sites such as liver and spleen.Virtually every organ can be involved by sarcoid lesions, but liver and spleen are the most often affected sites. Rarely pancreas, gastrointestinal and urinary system can also be involved with sarcoidosis.Although intra-abdominal sarcoidosis is usually asymptomatic, its presence may affect the prognosis and treatment options. The lesions are less characteristic and may mimick neoplastic or infectious diseases such as lymphoma, diffuse metastasis, and granulomatous inflammation, making its common features fundamental to be aware.
